To schedule treatment, Contact me. You may need a physicians order for treatment. However, Direct Access laws in the state of Tennessee allow you to come directly to physical therapy without a physician referral for a thorough evaluation and an individualized treatment plan. Patients are able to receive up to ninety (90) days of physical therapy treatment before your physical therapist must consult with your physician.
Your initial evaluation/treatment will be 1.5 hours and includes an evaluation as well as treatment. Cost is $200
Treatment after the initial evaluation costs $120/hour. Limited availability for home visits – plan on $1/mile to your home from my address (2289 Hurricane Creek Rd Linden, TN 37096) and back to my home in addition to the treatment cost.
Physicians order must have the following information. This allows you to submit a claim to your insurance should you want reimbursement for treatment:
- “Physical Therapy Eval and Treat”
- ICD code(s) (Diagnosis code from the doctor)
- Must be dated within 30 days of beginning treatment
- Signed by your physician
